Grace Hospital was the first certified Long Term Acute Care Hospital (LTAC) in Cuyahoga County. As a non-profit organization, our mission is to serve as a center for excellence in the provision of specialized health care services and comfort for patients and families coping with catastrophic illness and injury.
Grace Hospital is the only hospital-within-a-hospital LTAC in greater Cleveland, located inside University Hospitals. This unique setting gives our patients access to ICU transfer capability, onsite specialists, interventional radiology, and EPIC electronic health record continuity so referring physicians can follow patients across the continuum of care.
We care for medically complex patients throughout Cleveland, Tremont, Cuyahoga County, and Northeast Ohio who need hospital-level care for an extended period—typically 25–30 days. Our programs include ventilator weaning, inpatient wound care, complex medical and organ transplant recovery, and low-tolerance rehabilitation.
